Thank you for choosing Life’s Beautiful Beginnings.
Enrollment is quick and simple — just follow the steps below to get started in your selected program.
Step 1: Complete the Enrollment Form
Begin by filling out the Enrollment Form with your personal and referral information.
This form helps us collect your contact details and match you with the correct class.
Step 2: Complete the Intake & Assessment Form
After submitting your Enrollment Form, you must also complete the Intake & Assessment Form.
This step is required for all participants and allows your facilitator to better understand your background, goals, and referral requirements.

Step 3: Submit Your Payment
Payment is due before class begins.
You can submit payment through any of our approved methods:

Step 4: Review Class Schedule and Join via Zoom
Once both forms and your payment are received, you’ll receive a confirmation email with your class schedule and Zoom login link.

Important Reminder
✅ Both forms must be completed before you are officially enrolled.
